The overall purpose of this job is to provide high-quality support to the youth in the CCC Program in relation to improving the social, educational, and independent skills of the youth. To promote the well-being and safety of the youth in care.

Responsible To

CCC Director

Essential Functions:

  • Work in collaboration with the Operations and Clinical Team.
  • Will be required to obtain and maintain SCM certification throughout employment.
  • Attend supervision with Supervisor on a monthly basis.
  • Attend all required agency meetings.
  • Attend all trainings and meetings in regard to the youth on case load.
  • Complete intake process as required.
  • Create and maintain client general files.
  • Creation, maintenance and input of Life Book.
  • Complete pre-placement visitation of new youth coming into program on case load.
  • Complete all state required documentation on time:
    • 1294A; due on the 1st of each month
    • 1294B; due on the 15th of each month
    • 1293; per schedule; updated every six months
  • Complete monthly in home visit and monthly out of home visit.
  • Serve as a point of contact for all service providers, community partners, family and other collaterals involved in youthā€™s life.
  • Complete case management notes in the electronic medical record:
    • Documentation of conversations between DCBS, schools, community partners, health care providers, CBA and other service providers.
    • Documentation of in home and out of home visits.
  • Coordinate extracurricular activities for the youth and outings for youth in care.
  • Attend youth medical appointments and assist in transportation as required or deemed necessary.
  • Ensure youth have a safe, clean and conducive environment in the home.
  • Contribute to treatment plans.

Additional Duties:

  • Per management, may be required to work shifts in home as a TCSW as deemed necessary.
  • Attend training and development deemed necessary for this position.
  • Ensure that Health and Safety is observed in the course of employment.
  • Ensure compliance with Equal Opportunities Policy and Procedure in all employment.
  • May be required to assume other duties as may be requested by the Program Director/Program Manager that is commensurate with level of responsibility and skills.
  • Comply with Corporate Compliance.

Qualifications:

  • A Bachelors Degree in a Human Services related field.

  • 21 years of age or older
  • Valid Driver’s License

Work Environment:

This job primarily operates in a group home setting caring for youth with disabilities. You will be working as a team with other TCSWs to care for the youth in the home. Must be able to travel to our three locations: Lexington, Nicholasville, or Georgetown. However, one primary location will be assigned. Use of a company vehicle will also be provided. The personal vehicle will only be used in an emergency.

The incumbent may be required to stand and/or walk for long periods of time and occasionally climb or balance; and stoop, kneel, and crouch. The incumbent must frequently lift and/or move up to 50 pounds and occasionally lift and/or move up to 100 pounds.

This job demands the ability to pass and perform Safe Crisis Management at all times successfully.

While performing the duties of this job, the incumbent is regularly required to talk or hear. This jobā€™s specific vision abilities include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
