Therapist - Community Based

The behavioral health therapist professionally applies social work methods and theory to diagnose, prevent, and treat psychosocial problems, impairment, or disability, including mental, emotional, and behavioral disorders to families, individuals, groups, and organizations. The main responsibility of the therapist will be to conduct comprehensive assessments, set treatment goals, provide therapy, and arrange for expediting and coordinating services.

Essential Functions:

  • Participate in the delivery of care as scheduled
  • Perform and document patient referral, comprehensive assessment, progress in therapy and discharge plan.
  • Assist in the process of advocating for patientā€™s access to resources through community programs, referral to case management, and mentoring the patients, when appropriate.
  • Communicate treatment plan to patients, families, and appropriate staff.
  • Collaborate and coordinate with relevant parties to enhance client care (i.e., caregiver, physician, case manager, home health care, etc.).
  • Provide support groups and psychoeducation to patients on clinical issues such as anxiety, aggression, anger management, coping skills, stress reduction, and caregiving.
  • Instill confidence in client by demonstrating that they are working with an experienced professional.
  • Provide informal teaching to the Key Assets BHSO staff based on related patient psychosocial issues.
  • Provide community-based therapy services to clients within Fayette County and surrounding counties.
  • Communicate any child/adult protection matter or serious complaint to Director

Communication:

  • Act as a first point of contact for our customers and portray a positive, professional and helpful image of the organization
  • Ensure safeguarding and complaints are managed in line with procedures
  • Ensure all email communications to staff and external agencies are presented within the guidelines of the companyā€™s IT policy
  • Collect case studies and data to support marketing processes and procedures
  • Communicate with a wide range of audiences and deliver excellent customer service at all times
  • Attend and participate at team meetings
  • Present a professional image of Key Assets at all times to stakeholders and customers
  • Work with Universities to support research endeavors
  • Serve in the role of mentor and support as needed for new or struggling staff persons

Working in Partnership/Training and Mentoring

  • Attend and participate in team meetings

Additional Duties:

  • Attends and participates in training and development deemed necessary for this position
  • Ensures that Health and Safety is observed in the course of employment

  • May be required to assume other duties as assigned by the BHSO Program Director that is commensurate with level of responsibility and skills.

Required Skills & Competencies:

  • Proficient computer competency including knowledge of Microsoft Office suite and other systems or devices required for use
  • Proficient oral and written communication to include presentation skills
  • Proficient Interpersonal skills to include friendliness, a welcoming demeanor, professionalism, and cooperation
  • Proficient in planning, prioritizing, and managing time
  • The ability to work effectively, independently, and in a team environment

Qualifications:

  • Masterā€™s Degree in Social Work, Counseling, or Marriage and Family Therapy from an accredited institution required
  • Prefer experience using an Electronic Medical Records (EMR) System
  • Valid and active driverā€™s license with reliable transportation
  • Proof of auto insurance
  • Certificates/Licenses/Registrations/Credentials
    • Preferred – Licensed Clinical Social Worker (LCSW), licensed by the State of Kentucky
    • Other disciplines considered include LPCC and LMFT
    • Minimum qualifications – Certified Social Worker (CSW) licensed by the State of Kentucky and under supervision of an LCSW meeting Kentucky Board of Social Work supervisor requirements; LPCA licensed by the State of Kentucky and under supervision of an LPCC; MFT licensed by the State of Kentucky and under supervision of an LMFT

Work Environment:

While performing the duties of this job, services are provided as needed in the place where the client resides or needs the service. Case management is a community-based service and offers service in the client’s home, community, school, or place of worship, as needed. Case managers will provide linkage, monitoring, access to services, assessment and service planning, problem resolution, and informal support network building for assigned clients in a strengths-based, client-driven fashion where and when it is needed.
